Admissions Process 

Although patients do not need a prescription to see a physical therapist, many insurance companies require a prescription. Therefore we recommend that patients wanting to enroll in therapy at Berkana call their primary or specialty physician for a referral. Their physician can then fax the prescription to Berkana at 970.797.2509.

If you know that your insurance does not require a prescription, you can enroll directly by calling our office at 970.797.2431 for an appointment.
